Objective: To analyze the behavior of public spending on sick pay in Brazil, considering the diseases, gender, age, and region of the worker. Method: This is a retrospective longitudinal study, of exploratory descriptive nature, with a time frame of 2009 to 2017. A comparative analysis was performed regarding the characterization of the beneficiaries, including age, gender, region of the worker. Results: In rural and urban areas, it was observed that the highest expenditure occurs with men. As for the categorization by age group, the highest expenditure occurs with people between 55 and 59 years old, followed by the age group 50 to 54 years old. As for the reason for retirement, the main reasons identified were diseases of the circulatory system, diseases of the musculoskeletal system, neoplasms, and mental and behavioral disorders. Conclusion: It is observed that the amounts received by women continue to be lower than those received by men, that the urban population tends to receive more resources than the rural population, and as for age, older people receive higher amounts than younger people.
